CSF-PLAP using an 8 pg/mL cutoff demonstrates high diagnostic accuracy for intracranial germinoma. Recognition of potential false-positive results and consideration of sampling site may further improve clinical interpretation of CSF-PLAP measurements.

Giant seminoma of the testis (2.51 kg) in a setting of severe occupational and lifestyle exposures: a case report and pathophysiological insights. (PubMed, Front Med (Lausanne))
Delayed diagnosis due to occupational neglect allowed the tumor to reach an exceptional size. Although causal associations remain to be established, preventive strategies that reduce exposure to environmental carcinogens, address occupational stress, ensure adequate sleep, and promote routine scrotal self-examination and timely medical consultation may be considered on general health grounds.

Case Report: Mature cystic teratoma of the diaphragm in an adult male: a rare case and literature review. (PubMed, Front Oncol)
Isolated CA19-9 elevation may be associated with rare primary diaphragmatic teratomas. Furthermore, concurrent laparoscopic hiatal reconstruction (e.g., Nissen fundoplication) provides an excellent, structurally sound functional cure for large tumors intimately adhering to the diaphragmatic crus.

Case Report: Primary choriocarcinoma of the pineal region. (PubMed, Front Oncol)
The pathological findings were consistent with pineal region choriocarcinoma. The patient's serum β-HCG level decreased to <0.1 mIU/mL one year after surgery, and the child remained in good condition without recurrence during the 2-year follow-up.

HAVCR2 (TIM-3) as a favorable prognostic immune marker in seminoma: Integrative checkpoint screening and tissue validation. (PubMed, Cancer Treat Res Commun)
HAVCR2/TIM-3 expression demonstrates a consistent association with favorable clinical outcomes in seminoma across transcriptomic and protein-level analyses, with a lower risk of progression (HR = 0.201) in the pooled cohort. These exploratory findings support a potential prognostic role of TIM-3 reflecting an immune-active rather than exhausted microenvironment in seminoma, warranting prospective validation.

Challenges in Managing Primary Retroperitoneal Germ Cell Tumors in Children: The Role of Neoadjuvant Chemotherapy. (PubMed, Ann Surg Oncol)
This study confirms the favorable oncological prognosis of primary retroperitoneal GCTs in children. Neoadjuvant chemotherapy should be approached with caution, as its impact on tumor volume may be limited. Furthermore, it carries a potential risk of growing teratoma syndrome, which can ultimately lead to even more challenging surgical procedures.

Pembrolizumab Failure in an Aggressive, Platinum-Resistant Primary Mediastinal Yolk Sac Tumor With Rapid Metastatic Dissemination, Spinal Cord Compression, and a Fatal Outcome. (PubMed, Cureus)
We report the case of a 26-year-old man with a primary mediastinal yolk sac tumor who progressed despite multimodal therapy, including etoposide, ifosfamide, and cisplatin chemotherapy, high-dose carboplatin and etoposide with autologous stem cell transplantation, gemcitabine and paclitaxel, surgical debulking, and radiation therapy. No further disease-directed therapies were available, and he ultimately died from progressive metastatic disease. This case highlights the aggressive biology of platinum-resistant primary mediastinal yolk sac tumors, illustrates primary resistance to programmed death-1 inhibition, and underscores the urgent need for more effective salvage strategies in this high-risk population.

Very late extragonadal recurrence of testicular seminoma presenting as a bladder mass: a case report. (PubMed, Ann Med Surg (Lond))
It also emphasizes that normal tumor marker levels do not exclude recurrence and that histopathological confirmation remains essential for accurate diagnosis. Recognition of unusual presentations in long-term survivors of germ cell tumors is important to facilitate timely diagnosis and appropriate management.

The RNA helicase DDX53 (CAGE) contributes to RNA metabolism in a human germ cell model. (PubMed, bioRxiv)
Our data indicate that DDX53 protein acts as a regulator of RNA metabolism in human cells. Collectively, we show that DDX53 participates in transcriptome regulation (including splicing) in male germ cells and exhibits transcriptomewide RNA interactions, but its wider biological role remains to be clarified.

Treatment options in platinum-refractory male germ cell cancers: current standards and future directions. (PubMed, Ther Adv Med Oncol)
Platinum-based chemotherapy remains the standard of care in this setting, administered either as conventional-dose cisplatin-based regimens or as high-dose carboplatin-based combination chemotherapy. In this narrative review, we outline established and emerging treatment opportunities for this rare but challenging clinical scenario of platinum-resistant GCCs. This review provides a focused and up-to-date synthesis of therapeutic options for multiply relapsed, platinum-refractory GCCs, with a particular emphasis on late-line treatment sequencing and emerging biomarker-driven strategies, including CLDN6-directed therapies.
