Postoperatively, he received adjuvant stereotactic radiosurgery (30-35 Gy in five fractions) to the resection cavity and was transitioned from tamoxifen to trastuzumab deruxtecan (T-DXd) for improved CNS-directed systemic therapy. Surveillance imaging at 6 months demonstrated no intracranial recurrence, and systemic restaging showed no extracranial disease progression. This case highlights the importance of prompt neuroimaging for new neurological symptoms in male breast cancer patients and supports a multimodal strategy, including surgical resection, focal radiation, and CNS-penetrant HER2-targeted therapy, for achieving durable intracranial control in select patients with isolated brain metastasis.

PMRT was associated with improved OS in selected MBC patients with high-risk features such as larger tumors or PR-negative disease. The consistent findings from SEER and SJTUBCDB support the use of PMRT in high-risk MBC patients and highlight the need for personalized treatment strategies and prospective trials.

RS testing increased between 2010 and 2023 for both sexes, and RS influenced receipt of chemotherapy. Male patients had higher RS but were less likely to receive chemotherapy when RS ≥ 26, and when chemotherapy was omitted, they had worse OS, revealing the importance of chemotherapy among males with high RS.
The use of artificial intelligence (AI) and machine learning are emerging options that have the potential to improve detectability and personalized treatment in both genders. The current review draws similarities between breast cancer in males and females to promote gender-specific interventions and better outcomes.
